FINANCIAL POLICIES

 SCHEDULING – Appointments are held with a debit/credit card. At the time of service, you can alternatively use an alternate payment method or a gift certificate.

CANCELLATIONS – A $50 cancellation fee will be charged for cancellations or no-shows without 24 hour’s notice.

LATE ARRIVALS – Due to the tight schedule of our providers, your appointment will need to be scheduled if you are more than 5 minutes late for a Filler/Botox appointment and 10 minutes late for any other service. 

REFUNDS – All services are non-refundable, no exceptions. Money will not be given back after a service is given. If you are unsatisfied with your service, we will work together to find a resolution that is fair for both parties.

MEMBERSHIPS – The 22 Spa MD LUXE Membership is a 12-month agreement for $250 per month. Payments will automatically be charged to a credit card on file on the 5th of every month. Cancellation before the agreement is fulfilled will result in a $250 cancellation fee using the credit card on file. After the 12-month agreement is fulfilled, it will roll into a month-to-month membership that can be cancelled at any time with no fee. It is the responsibility of the member to book the service appointments. There is ONE rollover per year and it must be used the following month. Membership services can be gifted to a friend or family member but gifted monthly membership benefits do not include LUXE Membership discounts.

Membership services include one area only. Any additional area will be an added charge. Services included in LUXE Membership are: Lumecca, Microneedling, Chemical Peels, Forma Skin Tightening, Diamond Glow (face/neck), 22 Signature Facial, IV Therapy + Booster Shot, Clear and Brilliant. Discounts for LUXE Members include: $10 per unit Botox; $4 per unit Dysport; $100 off every syringe of filler; 15% off all services (excludes hormone therapy; 15% off retail product.

RETAIL PRODUCT – Unopened products in the original sealed packaging are eligible for return. In-store credit will be issued with all product exchanges and returns.

GIFT CERTIFICATES – All gift certificates are non-refundable and have no cash value.

All procedures performed at 22 Plastic Surgery are primarily cosmetic in nature, not medically necessary, and therefore, would be fraudulent and unethical for 22 Plastic Surgery to submit a fee to any insurance company for coverage. We will not accept insurance for any procedures.
